The Patient-Centered Medical Home emphasizes access,
communication and patient involvement, and has proven successful at
improving the quality of primary care, while lowering costs and
increasing patient satisfaction.

At the helm of the medical home model are primary care doctors
who demonstrate they coordinate all patient care with a team of
