We haven't switched anyone over yet. The sc drug was just approved and CMS has not yet assigned a J-code (usually takes +/- 3 months). Until this occurs, there is risk of commerical no payment if the miscellaneous J code (J3590) is submitted along with the invoice. Medicare spells out their rules
