the Allscripts customer base of more than 2,500 hospitals and
health systems access to Conversa's Conversation Platform™.
Conversa's solution will serve as the patient-facing touchpoint for
the Allscripts CareInMotion™ population health management
platform. The technology will also be offered as a standalone
solution. The collaboration supports Allscripts' patient engagement
and care coordination solutions, and brings enhanced value to its
customers.
Historically, it has been difficult for providers to identify,
communicate and manage patient populations efficiently. Results
from an American Hospital Association survey released
earlier this month underscore hospital and health systems'
increased adoption of technology designed to engage patients in
their healthcare and enhance patient-provider communication.
Representing more than 2,000 U.S. hospitals, forty percent
confirmed their organization uses real-time care management
services to provide virtual care for patients with chronic
conditions. The results prove provider organizations' recognition
of the need for tools that transform the way patients and family
members are engaged, educated, and empowered, and a heightened
focus on managing healthcare experiences across the care continuum
- in hospitals, ambulatory environments, clinics, physician
offices, and even at home.
